Caregiver Registration Requirements
Registered Nurses and Licensed Practical Nurses
- RN or LPN license
- Social Security Card
- Level 2 Background screening on file with AHCA
- Valid CPR card from American Heart Association, American Red Cross or American Health and Safety Institute
- Physical done within the last 6 months stating “free of communicable diseases”
- 1 hour of HIV/AIDS Certificate
- 30-hour Infusion Therapy Certificate (LPN only)
- Professional Liability Insurance – recommended
- Current Massachuttes Driver’s License
- Current Automobile Insurance and vehicle registration
Certified Nursing Assistants and Home Health Aides
- CNA License in Massachuetts (or HHA certificate – 40 hour minimum)
- Level 2 Background Screening on file with AHCA
- Valid CPR Card through American Heart Association or American Red Cross or American Health and Safety Institute
- Physical done within the last 6 months stating “free of communicable diseases”
- HIV/AIDS Certificate – 1 Hour
- Alzheimer’s Training Certificate – 1 Hour
- Self-Administration of Medication Certificate – 2 Hours
- Professional Liability Insurance – recommended
- Driver’s License
- Social Security Card
- Current Automobile Insurance and vehicle registration
