Caregiver Registration Requirements

Registered Nurses and Licensed Practical Nurses

RN or LPN license
Social Security Card
Level 2 Background screening on file with AHCA
Valid CPR card from American Heart Association, American Red Cross or American Health and Safety Institute
Physical done within the last 6 months stating “free of communicable diseases”
1 hour of HIV/AIDS Certificate
30-hour Infusion Therapy Certificate (LPN only)
Professional Liability Insurance – recommended
Current Massachuttes Driver’s License
Current Automobile Insurance and vehicle registration

Certified Nursing Assistants and Home Health Aides

CNA License in Massachuetts (or HHA certificate – 40 hour minimum)
Level 2 Background Screening on file with AHCA
Valid CPR Card through American Heart Association or American Red Cross or American Health and Safety Institute
Physical done within the last 6 months stating “free of communicable diseases”
HIV/AIDS Certificate – 1 Hour
Alzheimer’s Training Certificate – 1 Hour
Self-Administration of Medication Certificate – 2 Hours
Professional Liability Insurance – recommended
Driver’s License
